The protocol back then: a ruler, a flash, and eighteen photographs

The doctor in Caracas requested, as the book narrates, “serial photographic records, frontal, lateral, and oblique, with neutral lighting, plain background, and a metric scale for reference.” In practice, that meant two cold lamps, a white bedsheet as backdrop, and a plastic ruler held literally against the skin of a ten-year-old girl to give scale to every photograph — eighteen shots in total, of the face alone.

The goal was legitimate: without those images, the surgeon couldn’t plan anything remotely. But the method reduced the body to an object of measurement, something the book describes with total clarity:

What changed: how a burn is measured today

The clinical goal is still the same — knowing exactly how much body surface is affected, information essential for planning surgeries, calculating fluid replacement, and anticipating infection risk. What changed completely is the method.

1. From the physical ruler to a digital TBSA model

Today, applications exist that generate a personalized three-dimensional model of the patient, on which the doctor marks the burned areas directly, in different colors by depth. The system then calculates the percentage of affected body surface on its own, with no physical ruler ever touching the patient’s skin.

2. Several formulas, one automatic calculation

Instead of a doctor manually applying a single formula (like the classic Rule of Nines, designed for adults and less accurate in children), these systems compare several recognized mathematical methods — among them the DuBois, Mosteller, and Haycock formulas — to produce a more reliable percentage, adjusted to the patient’s real body proportions, including age.

3. Zooming into critical areas without new sessions

The same models allow zooming into especially delicate areas — hands, face — without needing new photo sessions or putting the patient through the process again every time a different specialist needs to review the case.

4. Fewer sessions, less exposure

Where a full photo studio session used to be needed — lights, backdrop, a photographer — every time the record needed updating, today a single initial capture can be reused and updated over time, reducing how many times a patient (often a child) has to go through that process.

Between the testimony and the protocol

None of this erases what Uyanic felt that afternoon. Today’s technology doesn’t solve the human part of being measured and cataloged — but it does reduce how many times someone has to go through it, and it restores some of the dignity a plastic ruler pressed against skin couldn’t offer.
